Book an appointment
Appointments can either be conducted by phone or face-to-face; each appointment is 15 minutes. If you have a lot to discuss please ask if a double appointment is available. As continuity of care is really important, please ask to talk to or see your 'usual' doctor, if possible.
From time to time you may be asked for consent to have medical and nursing students observe your consultation or to take part in research. This is, of course, always optional. On occasion, your notes may also be used in research but all personal details are removed.

Home visits
Please be aware that our GPs will always call to assess the home visit request first.
Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception before 10.30am.
Telephone: 0117 964 4343.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls.
You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under 5 years old.
Sickness certificates (fit notes)
You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.
Request test results
You usually don't need to book an appointment to get your test results.

Please remember to update your contact details with us when you change address, telephone numbers and email address.

Persistent defaulters maybe removed from the list.
